We have no idea what's going on

in gerald's finger.

The ct scan gave us a good

indication

that there's no real

involvement of muscles,

or bone or important nerves,

but you never really know

until you get in there.

Dr. James: Can you make a fist?

And stretch straight.

We've got a lot of nerve,

and artery and vain involvement.

So we're going to have to take

our time and look

at this in detail,

and be careful to maintain

the normal nerve

and blood flow function

for the finger.

So I don't want to find

that the connections

that are between the tumor

and the hand

are critical connections.

Large blood vessels that need

to be tied off or repaired,

and I'm not sure how big

they're going to be,

or how big

they're going to bleed.

You see to me to be

a little bit nervous.

You seem a little quiet

and reserved today.

Are you a little nervous

about anything?

I think with this general

anesthesia,

you're going to be

very comfortable.

The next thing you know,

you're going to be

in the wakeup room

feeling a lot better.

So I get to watch,

so I'm very excited about that.

what's going to come

One of the key considerations

for any tumor case

is recurrence.

So we've got to make sure

we get a good margin.

And we want to contain

that tumor and all the tissue

that we excise,

that we take out.

When removing this bump,

we don't want it

to certainly come back,

and we don't really

even know what this is,

so we want to remove it entirely

so we can at least ensure

that it is all out.

Dr. James: Okay, here we go.

we can see the vessels, but it's

a pretty vascular tumor,

and we're seeing that already.

So you would say that that's

pretty rare to get in the hand.

kind of a thing.

This is very unusual.

Okay, start going

into the minefield.

So if gerald's tumor is not

wellencapsulated,

that abnormal tissue

can spreout

throughout the normal tissues

of the hand.

That means it's tougher

to get rid of it.

The soft tissues of his hand

could suffer

if we have to take that out,

otherwise it's coming back.

It's just right

under there.

Thankfully, it looks like

a great capsule.

This is really good news.

That's great.

When dr. Lilley says there

is a good capsule,

that's a good thing.

It's like a really

wellcontained ball

under the skin.

It wasn't something that

was going to become a mess.

Dr. James:

But I want to make sure

we get those nerves identified.

How does a nerve look

compared to a vein?

A vein is kind of purple,

kind of red, dark blood,

blood that lacks in oxygen.

A nerve is really

something white, right?

Like, that.

Like, that.

Yeah, and it's

a white pale yellow,

and in the back of the finger,

it's super small.

And we got to take our time

and just kind of protecthem

as much as we can.

Like glass noodles.

That's right.

Okay.

So we've got something

going on in here.

This is the hard part.

This is the middle

of the minefield.

What are you saying, "wow?"

that there's a vessel there?

That looks like a nerve.

Yeah, that's a nerve

If you sever any

of these nerves,

you could lose sensation.

That nerve doesn't matter.

We don't need it.

This skin is going byebye.

Some of these nerves

are actually optional.

The one to the fingertip

is not optional,

so let's find that guy

before we take this out.

It can be nervewracking

to be taking apart

someone's hand

and putting it back together,

and you worry about accidents

happening.

It can be like a minefield.

It can be like

an unexploded bomb.

You know, is it the red wire?

Is it the blue wire?

So I don't want to interfere

or injure the nerve

on the palm side of his finger,

as that could leto numbness

of the fingertip

and his inability to feel things

when he pinches or grasps.

So this is the nerve

to the tumor.

We'll get rid of that.

And that...

Almost there.

...Really helps

that connection. Okay.

We're making good progress.

Those blue vessels look

threatening though.

They look so like varicosities,

it's going to be the toughest

and if I cut too much,

I'll be cutting into the normal

artery, and it's a disaster.

these nerves around this bump,

gerald could lose movement

in his middle finger.

He could lose sensation,

and that is a tough thing

to deal with

for the the rest of your life.

are sure what this is.

We've never really seen anything

quite like this,

and you need to see

what kinds of cells are there

to really get a firm diagnosis.

You also need to see what cells

are there to see

whether there are benign

or malignant cells there.

We'll really need to depend

on pathology to get a diagnosis.

Dr. James: Now, you have to trim

out the excess...

Looks good, yeah.

...And create some good flaps.

We need to make it look pretty.

I want to put the division

of the glabrous

and nonglabrous skin

in the sweaty

and the palmar skin...

Right.

So it looks natural,

it looks real, right?

...Hairbearing and

nonhairbearing skin naturally.

Especially in him,

he's darker.

He has a difference

in the color.

So to make sure

you don't want it...

It's a little distracting if

he hsome of his palmar skin

on the top of his hand.

You would be able

to see that.

And make sure we get

a good color match.

Whenever we take out

a tumor this size,

I want to make sure

we restore that balance.

And if we can keep the scar

on the very side of his finger,

it optimizes his function

and minimizes stiffness.

This is called

a congenital nevus.

Nevus is, like, the professional

name, really formal.

So usually when we see hairs

come out of moles,

it's probably because

you were born with that mole.

Okay.

Does it cause you pain

or discomfort?

Yeah?

What I personally don't like

about it

because I've seen many,

many other birthmarks,

but the things that

they don't have that I have

is the bumps and the hair.

That's what kind of worries me.

Okay.

A congenital nevus

is a type of mole

that essentially

you're born with.

A lot of times, these can be not

just on the surface of the skin,

but they can be

a little bit deeper.

They can extend down hair

follicles,

which is why we can

often see hair growth,

and there can be

a lot of bumps.

Though it is a mainly

cosmetic concern,

vincent does have a reason

to be a little worried.

You know, we all have moles.

In fact, most of...

The average number of moles

that we have,

in general, every person

You've taken the cake

with this one.

This is a bigger one.

Now, the thing that we're

concerned about with moles

is the fact that they may have

increased risk of skin cancer.

James has not seen my lump.

who is the world's sweetest

He does have a pretty large

congenital nevus on his neck,

but a mole like this could be

more than what meets the eye.

You've heard of the word

melanoma before?

That's probably the most

wellknown

but potentially life threatening

kind of skin cancer,

and so that's the kind

that we are the most cognizant

of as dermatologists

because that could

threaten your life.

We can certainly take a little

sample there

and make sure

it's nothing to worry about.

Jessica: I'm a little nervous

that it could be cancerous.

I'm just glthat she's able

to do the biopsy

so that way it if comes back

that there's something,

at least we caught it in time.

The thing is that the smaller

a mole is like that,

the easier it is to remove,

right,

because you actually have

to pull that tissue together.

This sort of thing can be

removed, but it is not something

that is going to be able

to be done in one sitting.

Sometimes you have to do

what's called tissue expanders,

something that stretches

the areas

of another part of your body,

so you stretch

that skin a little bit.

You can't pull that sidetoside

there, you know,

and edgetoedge and be able

to move your neck around.

There's not enough

skin there.

It'smportant for vincent

or for anybody to understand

the commitment that is involved

because this is a long,

laborious, painful process,

but, you know,

you're not making it go away.

You're really changing it

into something else.

Laser hair removal is not

really a great option here

because it's trading color.

So if you have white hairs,

it's really not going to

get rid of them,

but if there areas that are like

raised like this here,

right here and right here,

I think we should take...

We can take those two off

right there, certainly.

Did this always bother you,

even when you were pretty young?

It mostly bothered me whenever

I just started school

because people really

didn't know me,

so people just kept on asking

and asking, "what is it?"

then after, like, fifth grade,

sixth grade,

everyone just didn't

even care.

They just became my friend.

You know, yeah, people are

going to ask questions.

People ask questions

but I think this makes you
